There is free street parking in front of the building. It is also accessible through public transportation. There are no stairs to enter the building or the office itself.
Appointments
are available on Mondays, Tuesdays, Thursdays and Fridays 9 - 4.30. (last appointment starts at 3.30). Appointments can be made via email or phone.
Cancellation policy
Please note that a 24 hour cancellation is required. A $50 cancellation fee will be charged for late cancellations. This fee cannot be charged to insurance and will have to be paid by check or cash before continuation of therapy sessions.

Can I use my health insurance?
Myofascial Release is a form of Physical Therapy and is covered by your health insurance accordingly. Your charges will be billed directly to your insurance company.
You are, however, always responsible for any portion of your charges insurance doesn't cover. Lilian Holm Physical Therapy accepts no responsibility for knowing what your insurance does and does not cover. Please familiarize yourself with your policy, and bear in mind that you are responsible for payment of your services.
What insurance policies do you accept?
I am an in-network provider for Blue Cross/Blue Shield PPO, and an out-of network provider for other private insurance carriers (Aetna, United Healthcare, Cigna etc). I also accept Medicare.
What if I don't have insurance?
Many patients pay for their services on a self-pay basis. Please
contact me for details.
Do I need a prescription?
If the cost for your therapy will be covered by insurance this depends on your individual insurance policy. BCBS policies typically don't require a prescription from a doctor. If your therapy will be covered by another type of insurance you may need to obtain a prescription for Physical Therapy from your medical doctor, osteopath or dentist. Contact your insurance company directly by dialing the number on the back of your insurance card to find out what their requirements are. You may not have to visit your doctor for a prescription; it can simply be faxed directly to 847-492-0321.
All patients, including self-pay patients, need a documented diagnosis. This could be in the form of verbal, electronic or written communication from your doctor or dentist, a copy of a chart note or a prescription.
What can I expect from my treatment session?Your first treatment session will include a medical history and a detailed examination of your alignment and your myofascial tissues. You will learn a lot about the factors that are contributing to your symptoms, and what can be done to remedy the situation. Your treatment will be outlined, and you'll start learning about what you can do for yourself as well. You will also typically receive a shorter treatment session on your first visit, and from your second visit onward your treatment will consist of at least an hour-long hands-on session.
What do I need to bring to my first visit?Please arrive about 15 minutes early for your first appointment to fill out some paperwork (we want to spend the full hour examining you, not papers!) that you will find waiting for you in the waiting room, located to your immediate right as you enter the building. Flick on switch #4 on the wall in the waiting room as you arrive.
Bring any test results you may have (e.g. MRI reports, bone scans etc, and any X-ray Plates or X-ray reports.
Bring a pair of soft shorts or the equivalent thereof for the evaluation and treatment. Ladies can simply wear their regular bra as well, which is much preferable to a sports bra, as it covers much less of your upper back. If you religious or other personal preferences prevent you from wearing shorts or dressing lightly in general, this can be acomodated, but please be aware that the purpose of dressing lightly is two-fold: for the therapist to be able to see your physical structure, and to enable the treatment, which is done on bare skin. Please bear this in mind in order for your treatment to be as pleasant and effective as possible.
Please bring your prescription, if you have one, and your insurance card if you'll be using insurance. You may want to familiarize yourself with your insurance policy and specifically your physical therapy benefits prior to the appointment.
Lilian Holm Physical Therapy claims no responsibility for knowing what your particular insurance policy covers or doesn't cover.
